Use this form to send us the money you’ve raised by cheque or card. There’s also space to tell us about your event and why you chose to fundraise for Coeliac UK. Please don’t send cash in the post.

Sponsorship Form
Print this out to collect sponsorship for your event or challenge. Ask each UK taxpayer to tick the Gift Aid box so we can claim an extra 25p for every £1 they give, then send the completed form to us when you pay in.
